We are "in network" with:

- Ohio Medicaid

- United Healthcare

- We are also certified with TriCare, but may not be considered "in network."

Please have your card with you at the time of your visit. If we are able to file your claim, you are responsible for only the co-pay at the time of the visit.

If we are not a provider for your insurance, if you are not insured, or we do not file with your insurance company, you will be responsible for all charges at the time of service. We accept cash and all major credit/debit cards.  We do offer a "sliding scale" reduced fee structure for lower income families without adequate coverage who choose to provide proof of income/tax filings.

We are happy to bill other insurance if we are "out of network," but you will likely incur higher out-of-pocket charges (check with your carrier for more info).
